It is recommended that I seek a licensed health care professional for any physical or psychological ailment that I have. If I am experiencing a medical or psychiatric emergency, I will call 911 or go directly to an emergency room. 

​I understand that there is a $50 fee if I need to cancel within 24 hours of my booked appointment.
